I like the "3 strikes and you're out" kinda system that rachelnlucas mentioned, IF the teammate killed decides to. It's their choice if they want to kick you. If you noticed them pulling the trap on you just because you're fighting an enemy 1 on 1, then yah kick them. That's annoying. Getting a death from a friendly killing you should stay. It is a death, after all.
I don't like how it says "d1v4d killed by d1v4d" when Polyphemus kills you. I found that odd.

Implementing a punishment system for this just won't work. People will abuse it and kick people for any reason. The game itself won't have AI smart enough to know if a player is being detrimental to their teammates, so that's essentially why you need mics to work properly.
You pretty much have to put them on blast and the only place you should be able to punish them would be the lobby if they don't run off after the match.
The bigger issues is that people play this game and have no idea what they're doing or who is on their side, and then you have those who purposely take pleasure in sabotage.
You just have no way to separate the two.
