Mass Effect 3 Insanity trophy glitch?

I have had two of my friends play and beat ME3 on insanity and they both did not unlock the trophy for doing so. I was wondering if anyone else has dealt with this problem too? I am also about to jump into insanity and try and get my last trophy for the platinum but  I don't want to get the same problem of not unlocking the trophy.

Didn't have this problem. Changed the difficulty as soon as possible on Earth, got the trophy when I beat the game. Platinumed in one playthrough (thanks to multiplayer).
