I turned on my PS3 this morning to install some of the stuff that I started downloading through PS+, and saw that there's a ~500MB file labeled 'METAL GEAR SOLID 4 GUNS OF THE PATRIOTS' (in all caps, for whatever reason, and missing an icon) in my Download Management queue. Is this a patch or something?

there was a 2.0 patch released a few months ago that removed MGO and added trophy support.

I wonder why it didn't start downloading until just now. I suppose the trophies aren't retroactive, are they?

nope. have to start a new game to earn trophies.
