Give this a shot everyone.
To restore the headset to its factory default settings:

I've just now got this problem. I got the PULSE Headset just a month back (imported from Hong Kong, as this headset isn't available in India - so I'm jacked to get a replacement) and everything was going fine. The trouble started tonight when I just wanted to add a 'Mode' that they have on the PULSE Manager App, from one of the presets to 'Bioshock'...
At the loading stage (where you press START and do the update) the headset would disconnect at 24%, then re-connect.
I repeatedly followed the same procedure to update the modes and it just kept disconnecting at 24%.
In the end I just gave up and closed the app, tried the headphones.... NO SOUND.
The PULSE headset is stuck in Connection Standby mode (double blue flash)
I follow the PULSE manual and reset the dongle with a pin, then do the MODE/ MIC-MUTE/ VSS turn the power on...
The headset went into 'pairing mode' (single blue flash). The dongle went into 'Connection Standby mode' (double blue flash) before the light turned off. Again I tried this a few times and nothing.
Then i re-plugged the Headset via USB back into the PS3 and from the PULSE menu tried the 'Factory Reset'.... again the headset disconnected at 24% disrupting the update, then reconnected. As some other posts have mentioned, I then got "an unknown USB device has been connected" coming up on the PS3 screen.
Now the Headset isn't even flashing! **bleep**?
And why would the headset always disconnect at 24% through the Manager app???
Every Sony recommended route to fix this problem seems to have just made it worse. Unbelievable.
Thank God I found this page, as Sony has NO INFO on this and Google comes up pretty blank... luckily they have indexed enough of this thread, so that PULSE HEADSET PROBLEMS for example throws some light on this.
I apologise now if this is a long message, but I'm hoping that maybe there's something here so Goggle can index thoroughly enough to help next poor **bleep** who's bought this **bleep** headset.
I guess the PULSE Manager App is reason this has happened. I guess avoid it at all costs if your PULSE is working..
I'm gonna follow the tip to leave the headset on overnight and drain the battery, maybe something comes of it.. Let's see.
