I say Normal really. The enemies are not different. Hard might be more realistic since you will die faster. But ultimately its a game and well, you can die on normal too if you screw up although I haven't died much at all (perhaps twice only). But its because I played the living hell out of the demo so I know sort of what I can do and what I can't (despite the demo being infected only xD).
But you can change difficulty mid-game. If you play very thoroughly: Hard might be the best for you. Right now I feel pretty loaded with ammo in Normal.
EDIT: Although for the most realistic: I'd say Hard. I imagine you die rather fast from bullets there xD
I think that had to do with the fact that Joel just aquired a gun and he saw him reload it which made him flee.
It feels like the AI takes this into account because like I stated above: I am loaded with ammo and such stuff xD. But I have seen runners for instance: Run and gather friends and then come back and get me.
Also at one point, I hit this guy with the butt of my pistol and his friend saw it and went running. I chased him down as he was heading to a room that I later found out to have two guys in it. It felt like he was going for reinforcements or something here too. In short: It feels like it could be possible.
The AI is really outstanding. It's hard to predict what they do and when they do something, you tend to be amazed.
It also depends on how well you play. If you go guns blazing and they have the numbers on you, they might just all go for it. But if you capture one of them, and play a bit with you know.. real tension and drama in mind: You sure will be in for a treat!

Unbelievable. I feel everyone has this but me now. You must be excited as hell. The **bleep******* here in Germany wont give you the game a second earlier than the original release date. I really need to stop preordering at gamestop
The only negative I could say is that the friendly AI has like once or twice been a bit slow to follow me and it felt like the enemy AI had seen them but they didn't react. This is good because the game doesn't technically punish you for what is the games own mistake. So in reality: It's just a bit of a hit on the immersion but nothing major. Like seriously, if that bothers you, you're weird xD.
Even in the E3 demo when Ellie comes into cover, you kinda see the bad guy look right at her. So it's nothing new. They are helpful as hell though and the game doesn't feel like an escort mission at all. It's lovely.
Also let me tease you by saying:
The intro of this game.. you won't see it coming.
